Government failing to act on recommendations

Today (15 July 2025), Louise Haigh MP has called on the Government to urgently act on implementing recommendations made in Domestic Homicide Reviews (DHRs), which are case reviews carried out when anyone over the age of 16 is murdered in a domestic setting.

The call to action follows the publication of a damning report this week by England’s Domestic Abuse Commissioner, Dame Nicole Jacobs, which has found that half of the national recommendations made in DHRs are not put into action, with only a quarter fully implemented.

The report reveals a deeply concerning lack of oversight, at the top of government, with women’s and domestic abuse charities urging the government to prevent further domestic abuse related murders and call for more accountability and central enforcement on this issue.

Currently, Home Office guidance says these reviews, which make recommendations to help prevent people from being killed by partners or family members, should be finished within six months. However, recent analysis has found that most reviews are taking years to complete.

In Yorkshire and Humber, 97% of reviews were not completed within the six-month timeframe, meaning that recommendations to prevent further deaths are delayed in the system even further.

Louise Haigh MP for Sheffield Heeley, said: 

“This report by the Domestic Abuse Commissioner is damning and clearly letting victims and their families down, time and time again.

“From domestic homicide reviews, to inquests, to delays in Child Safeguarding Practice Reviews, recommendations are routinely ignored.

“These reviews provide crucial lessons and warnings but are far too easy for Government at all levels to dismiss.

“It’s clear we urgently need independent oversight and enforcement to ensure that no more lives are lost in vain, and to honour the wishes of victims and their families.

“These tragedies are completely avoidable, and this government urgently needs to do more to make sure lessons are learned and no more recommendations are lost.”
